Perhaps Rieko's shrinking came about by a chemical reaction between otherwise harmless food additives, the cold meds and water treatment chemicals. The quantities she consumed was in just the correct ratios to cause organic shrinking/minaturisation. They both realise that by going to any medical professional risks Rieko will become a test subject for experimentation, dehumanised and kept caged.

Yuta, apon discovering this doll is actually a live, living person (his mother) sets about making her his pet and toy, according to his youthful perception.
Yuta subjects Rieko to a series of humiliating exercises and 'handlings' to bend her will to obey him.
